Key Points
- It is distributed as a BRIGHT PINK, cardstock, multi-page form, but it can be photocopied and faxed while remaining a medical order and legally binding.
- It may replace a NYS non-hospital Do Not Resuscitate (DNR) form.
- The NYS non hospital DNR form is still valid.
- It includes information to be used in other health care settings such as the hospital (i.e. placement of feeding tubes, and so on).
- Remember to ask is the MOLST form is present.
- Add "D" to the SAMPLE acronym:
- S - Signs and Symptoms
- A - Allergies
- M - Medication(s)
- P - Pertinent past medical history
- L - Last oral intake
- E - Events leading up to contacting 911
- D - Do you have advance directives and medical orders
