Medical Orders for Life-Sustaining Treatment Manual

MOLST ManualThe MOLST Program: A Community Approach to Improving Care at the End-of-Life is an all-day conference encompassing the full scope of the advance care planning process.  Patricia Bomba, M.D., a national expert, will lead the conference in patient and system-based approaches to implementing traditional directives for healthy adults and the MOLST Program for seriously ill adults.  The conference will address the importance of effective communication, documentation of medical orders on a common community form, and ensuring health care providers are trained to honor patient preferences.

We have a revised MOLST Manual.  Please check back shortly, as the contents of the MOLST Manual will soon be available online.  We will be sure to advertise this information in the Bomba Letter and the MOLST Update once the MOLST Manual is here.

You can receive a complete MOLST Manual if you attend an all-day MOLST Conference.  Please view the list of upcoming MOLST Conferences on the Events page.  The complete MOLST Conference agenda can be found below.

WANTED: MOLST Trainers
We are creating a directory of certified MOLST Trainers throughout New York State.  The directory is meant to provide New Yorkers with access to those individuals trained in the MOLST Program who are willing to do training on advance care planning and the MOLST Program for others in their local area.  We need individuals for both professional training and community education.  If you are interested in becoming a MOLST Trainer or have been doing MOLST Training and want to be listed in the new MOLST Trainers Directory, please complete the MOLST Trainer Information Survey.
