The MOLST DVD is a statewide initiative presented by the Medical Society of the State of New York and Excellus BlueCross BlueShield in collaboration with the New York State Department of Health and the Monroe and Onondaga Counties MOLST Community Implementation Team.

Continuing Education Credits

This activity has been implemented in accordance with the Essentials Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through joint sponsorship of the Medical Society of the State of New York and Excellus BlueCross BlueShield. The Medical Society of the State of New York designates this educational activity for a maximum of 2.0 AMA PRA Category 1Credits.

For nurses, 2.0 Continuing Nursing Education contact hours are provided. The University of Rochester Medical Center/Strong Memorial Hospital and Strong Health Affiliates is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation.

For Case Managers, 2.0 clock hours are approved by the Commission For Case Manager Certification (CCMC).  Excellus BlueCross Blue Shield is the sponsoring organization for these clock hours, designated as a provider of continuing education for certified case managers by CCMC.

EMS Providers

EMS providers should view Video 1and MOLST Comprehension Review or use the EMS presentation at Print Documents. The Post Test Evaluation will provide useful data for the MOLST Community Pilot Project.
